Provide program coordination, support group facilitation and case management for individuals in Outside In’s Medication Assisted Treatment (MAT) Program. Work closely with the physicians prescribing Suboxone to develop and administer consistent service protocols and supports. In addition, provide harm reduction counseling to injection drug users. Assist with referrals and connection to healthcare and treatment resources as needed. This position is based in a Federally Funded Health Center (FQHC) in downtown Portland serving adults experiencing homelessness, substance abuse and mental health concerns, and chronic illness. This position requires some hours stationed in Intravenous Drug Use Health Services (Needle Exchange).
This position is 40 hours a week with full medical and dental benefits and a generous paid time off program. Pay is $16.45/hr.

• Provide initial program screenings within primary care, and provide ongoing brief interventions with MAT clients in coordination with primary care provider
• Provide information and referral for substance abuse treatment in the community
• Co-facilitate support groups for patients in the Medication Assisted Treatment program
• Further develop and coordinate Medication Assisted Treatment program including processes for referral, screening, induction, and ongoing follow up
• Work as a member of an interdisciplinary care team in close collaboration with prescribing primary care providers, pharmacists, and other Outside In staff
• Assist with scheduling MAT appointments, respond to client phone calls
• Complete administrative tasks related to the screening, eligibility determination, assessment and quality provision of MAT services to include pharmacy coordination, insurance issues, and prior authorizations
• Further develop and maintain tracking system to monitor Suboxone prescribing

• BA in social services related field; CADC preferred
• At least 2 years of experience providing counseling, case management, and/or treatment services (dual diagnosis preferred)
• Ability to appropriately manage crisis situations
• Program coordination experience preferred
• Experience working with homeless and/or injection drug users
• Knowledge and understanding of stages of change theory and trauma-informed care
• Working knowledge of Harm Reduction and Motivational Interviewing
• Excellent organizational skills, attention to detail, and data collection
