CAP is currently hiring a CareLink Team Lead to manage CAP’s CareLink program. CareLink provides high-quality, strengths-based short-term case management for individuals living with HIV who are not currently engaged in HIV medical care. CareLink primarily assists persons living with HIV (PLWH) who:
• are newly diagnosed with HIV and need additional support to engage in HIV related care
• have been out of medical care for more than 6 month and need support to reengage in care
• are exiting Corrections Facilities and needs assistance engaging in HIV services
• at high risk of becoming disconnected from medical care due to homelessness, substance use and/or mental health disorders.

Key responsibilities include:
• Managing a shared caseload of high-acuity participants in collaboration with the CareLink Navigator team
• Establishing rapport with participants and working with them to develop individualized goal plans to facilitate participants’ engagement and retention in HIV medical care, substance abuse treatment and/or mental health services
• Developing and maintaining relationships with key community partners including federal, state, and local corrections institutions and Ryan White Part A service providers such as medical case managers and HIV medical providers in order to ensure streamlined linkage to care and support services for PLWH
• Developing and coordinating culturally-informed participant outreach, recruitment, and engagement efforts
• Conducting routine quality assurance such as participant file audits in order to ensure accurate and complete data entry, current Ryan White certification, and CAREWare eligibility determinations for all CareLink activity and staff
• In consultation with the Manager of Prevention Services and the Director of Healthcare Operations, the CareLink Team Lead hires, manages, coaches, evaluates and supports employees under their direct supervision (currently two staff totaling 1.8 FTE).

We are looking for an enthusiastic individual with a strong background in supporting people who have been incarcerated, are experiencing homelessness, mental illness, and/or struggling with substance use. This is a full-time 1.0 FTE position which reports to the Manager of Prevention Services and is based in our Portland office, but will travel throughout the six-county service area for community partner meetings, accompanying participants to appointments, participant home visits, and outreach at local, state, and federal corrections institutions. Evening and weekend work is occasionally required.

Required Qualifications:
• Four (4) years equivalent professional experience (i.e. community health, social services, insurance, etc.) or Two (2) years’ experience of relevant work experience combined with a Bachelor’s Degree in human/social services field (social work, public or community health, psychology) or related field
• A minimum of one-year experience providing case management services to people who have experienced homelessness, mental illness, and/or substance dependency
• Experience working with ethnic, racial, economic and sexually diverse populations and strong understanding of issues relevant to PLWH and the LGBTQ community and how those issues affect or influence service delivery
• Experience with assessments, goal planning, and crisis resolution
• Knowledge of the principles of Trauma Informed Care and integration of Trauma Informed Care into service delivery
• Demonstrated ability to effectively collaborate with community stakeholders
• Excellent organizational and communication skills
• Ability to work independently with accountability; exercise sound judgment and discretion
• Available to work occasional evenings and weekends
• Demonstrated computer and keyboard proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet

Preferred Qualifications:
• Experience with monitoring and achieving program deliverables, report writing, and working with a leadership team to accomplish program objectives
• Experience with Service Point and CAREWare
• Master’s degree in social work or related field
• Experience managing paid and/or volunteer staff
• Experience working with persons exiting County, State or Federal Corrections
• Knowledge of HIV and non-HIV-specific social services in the Portland Metro Area
• Previous professional or volunteer experience working with people living with HIV
• Verbal and written fluency in English and Spanish
