Seniors Program Coordinator .4 FTE / Seniors Case Manager .2 FTE

Part-time Position (24 Hours a Week) Non-exempt

Reports to: Community Services Program Manager



POSITION SUMMARY:

Provide a culturally specific environment for seniors to meet, share meals and engage in activities. The Coordinator is responsible for providing the opportunity for seniors to connect with the larger community by raising awareness through educational presenters, resource materials, and participating in county-wide policy discussions. The Coordinator also provides case management services to Latino older adults with a focus on a family caregiver program.



PRINCIPLE DUTIES AND RESPONSIBILITIES:

Nutrition & Activities Coordination

Coordinate, provide, and oversee structured meals and activities four days a week (Monday- Thursday from 11am-12:30pm).
Follow the Oregon Congregate Nutrition Standards complying with the USDA Dietary Guidelines for Older Americans.
Meet the stated monthly goals of meal service and activity service provision.
Include participant voice in program development and delivery.
Conduct research to identify presenters and topics of interest to older adults.
Coordinate and facilitate logistics for the delivery of Evidence Based Health Promotion classes/workshops.
Recruit, coordinate, and supervise volunteers to assist in the program.
Conduct outreach to increase senior participation in activities and meal programming.
Maintain appropriate required documentation of service delivery.
Enter data in a timely manner into Salesforce (EPHC database), Oregon Access, and Compass that adhere to all EPHC and County Guidelines.
Work closely with seniors' program staff to ensure all safety, contractual, and agency requirements are met.
Actively participate in various team projects including team, agency, program and partner meetings.
Adhere to EPHC policies and procedures.
Performs other duties as assigned.


Case Management

Provide case management to unpaid caregivers in Multnomah County.
Assist caregivers with respite care funds based on their needs.
Conduct an assessment to identify needs and develop service plan in partnership with the participant.
Identify the appropriate referrals and services needed to meet the goals of the participant.
Advocate for the needs of participants (with for eg: health care providers, family members, medical settings, and other organizations, etc.).
Conduct home visits as needed.
Enter data in a timely manner into Oregon Access (Oregon State database), and Salesforce (EPHC database).
Maintain confidential participant files in compliance with EPHC and Multnomah County.
Participate in monthly file reviews.
Perform other duties as assigned.


QUALIFICATIONS:

Must be bilingual and bi-culturally competent (Spanish/English).
Knowledge and experience providing services for older adults.
Experience with community networking, organizing, and event/activities coordination
Bachelor's degree in social work/human services preferred; and/or equivalent work experience.
Certification on Stanford Chronic Disease Self-Management Programs: Tai Chi for Better Balance, Diabetes Prevention, Powerful Tools for Caregivers, and Tomando Control de su Salud. Preferred but not required.
Proficiency/fluency in written and oral communication in both Spanish and English.
Ability to build positive and trusting relationships with diverse populations,
Ability to work well in team setting, as well as independently; be flexible and adapt well to different dynamics in a fast paced work environment.
Must be able to organize and prioritize work, be proactive, take initiative, follow through, and simultaneously manage multiple priorities to ensure goals are met in a timely manner. High attention to detail required.
Demonstrate judgment and discretion in dealing with confidential matters.
Willingness to learn new skills and take on new responsibilities.
Ability to lift 40 pounds.
Must have driver's license, access to vehicle and ability to obtain automobile insurance at levels required by agency (100/300/100).
Successful completion of civil, criminal and motor vehicle background checks.
Ability to support EPHC's mission and goals.


COMPENSATION: Starting pay rate is $17.00/hr. Compensation is commensurate with skills and experience.
