Reports to: Youth Services Program Manager



POSITION SUMMARY:

Using a comprehensive culturally specific community health approach, the Warriors of Wellness Community Health Worker (CHW) provides case management, navigation, outreach and health education services to Latinx individuals and families in the community.



PRINCIPLE DUTIES AND RESPONSIBILITIES:

Carry an ongoing caseload of 12-15 individuals (for a total of 40 annually) who will be receiving case management services.
Comprehensively assess strengths and needs of individuals and families receiving case management services.
Provide "soft touches" or one-time service and/or referral to 50 individuals annually
Support Latino households with pre-diabetic conditions or diagnosed with diabetes to access services that help them to improve their quality of lives.
Actively participate in the development of an annual work plan that will serve as the blueprint of the work to be completed throughout the year.
Conduct outreach in the community to connect individuals and families with social and health services
Organize, coordinate and facilitate culturally specific health promotion groups and activities to increase understanding on how to access health care services and improve health management skills.
Maintain qualitative and quantitative data for monthly reports and quarterly progress reports as required by the agency and funder (Salesforce and the health information technology system provided by ORCHWA)
Host health services/screening in community settings (schools, places of faith, community centers etc.)
Support the organization and maintenance of updated resource materials for client needs.
Keep accurate and up-to-date documentation in files.
Participate in community and partner's meetings as indicated by program manager.
Attend a monthly group clinical supervision and individual clinical supervision meetings
Adherence to El Programa Hispano Católico policies and procedures.
Perform other duties as assigned.

QUALIFICATIONS:

Must be a certified CHW or have a plan to complete CHW certification within the first year of employment.
Must be bilingual (Spanish/English) and bi-culturally competent.
Ability to communicate concisely and effectively, both verbally and in writing in Spanish and English.
Bachelor's Degree or 2-4 years' or equivalent experience required.
Experience with application of Popular Education Theory.
Experience working with a broad range of issues affecting the Latinx community.
Demonstrate excellent judgment and discretion in dealing with confidential matters.
Intermediate skills in Microsoft Office programs (including Word, Excel, Outlook and PowerPoint).
Must be able to organize and prioritize work, be proactive, take initiative, follow through, and simultaneously manage multiple priorities to ensure goals are met in a timely manner. High attention to detail required.
Ability to work well in team setting, as well as independently; be flexible and adapt well to different dynamics within a fast paced environment.
Ability to take initiative and prioritize tasks; good time management, problem prevention, and problem solving skills.
Ability to work a flexible schedule, which could include some evenings or weekends.
Ability to support organizational and program specific mission and goals.
Successful completion of civil, criminal and/or motor vehicle background checks.
Must have driver's license, ability to obtain automobile insurance at levels required by agency (100/300/100). Frequent driving in the Tri-County area required.
