TITLE: LEAD Case Manager I
DEPARTMENT: Community Engagement Program/LEAD
REPORTS TO: Program Supervisor
SCHEDULE: Monday-Thursday, 9:30am-8:00pm
SALARY: DOE, Hourly
SUMMARY: The LEAD Case Manager works as a member of a multi-disciplinary team providing comprehensive assessment, individual planning, street outreach, intensive case management, peer services, and service brokerage. The team coordinates with community resources to provide access to address all needs, including crisis management, substance abuse treatment, medication management, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing, accessing employment and educational opportunities, and accessing supports for basic needs. The LEAD Team employs Case Managers with varying levels of experience, training, and education in order to assemble a diverse and competent team.
1. Completion of High School or GED.
2. CADC I, CRM, PSS or registered preferred. Ability to obtain CADC I, PSS, or CRM within six months of hire and a combination of at least two year’s work, education, training or experience specializing in substance use or mental health treatment, outreach, or case management.
3. Must meet CCC privileging requirements as required by FTCA.
4. Has the competencies necessary to:
a. Communicate effectively
b. Perform assessments and develop service plans,
c. Understand treatment and service terminology and able to apply the concepts
d. Provide psychosocial skills development; and
e. Implement interventions and broker services prescribed on a service plan.
5. If in recovery, must be able to document continuous abstinence under independent living conditions or recovery housing for the immediate past two years.
6. Must pass a pre-employment drug screen, TB test, and background check. 7. The OAR prohibits program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
8. Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
9. Must possess a current drivers license, pass a DMV background check and be designated an “acceptable” driver as outlined in Central City Concern’s Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
10. Required to be available for after-hours pager and holiday coverage rotation.
11. Must adhere to agency’s non-discrimination policies.
12. Ability to effectively interact with co-workers and individuals with diverse ethnic and cultural backgrounds, religious views, life-styles, and sexual orientations and treat each individual with respect and dignity.
