Family Support and Counseling Services
Full-Time, 40 hrs; Non-Exempt
Reports to: Director, Family Support and Counseling Services
The Engagement Specialist serves as an advocate and first point of contact for individuals and families seeking services in the Family Support and Counseling Services Division. This position will serve as the primary team member responsible for screening and assessing for readiness for services, works to address any barriers to accessing services, and makes referrals for additional resources as appropriate. The Engagement Specialist is responsible for coordinating resources and referrals within Family Support and Counseling Services and to community assistance programs. The Engagement Specialist is an integral member of the team and collaborates with the Clinicians, Case Managers, Psychiatrist, Peer Supports, Billing Coordinator, and other interdisciplinary team members for individual and family healing and wellness.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
Works within a strengths-based, trauma-informed approach.
Handles all incoming calls, assesses caller's needs, provides information about available services, and makes referrals as needed.
Assigns provider based on the individual's or family's need and schedules intake appointment.
Welcomes individuals and families to the Center and orients them to the services.
Screens, intakes, and supports individuals or families as they prepare to enter services.
Identify and address barriers to client attending services.
Provides phone outreach in collaboration with the provider to re-engage individuals and families with missed appointments.
Will serve as a liaison between agency and community resource agencies.
Will collaborate with direct clinical providers, utilization review organizations, and billing coordinators regarding needs, referrals, and service coordination.
Will maintain a current resource library and database.
Participates in the development and implementation of new programs, operating protocols and communication initiatives related to service provision.
Contributes to goal setting, process improvement and achievement of desired team outcomes.
Meets standards for documentation and performs data entry related to care, assistance, community referrals and quality indicators in the electronic record according to established guidelines.
Provide supervision of volunteers and administrative interns.
Participate in trainings and coordination meetings as requested by Supervisor.
Adherence to Catholic Charities policies and procedures.
Perform other duties and responsibilities as assigned.
Associate's degree or a minimum of 2 years related work experience.
Knowledge of available community resources strongly preferred.
Ability to communicate with direct care providers and community agencies appropriate for follow up.
Respect for cultural diversity, religion and experience and an understanding of human developmental needs.
Excellent written and oral communication skills.
Ability to interact in a respectful, professional manner in accordance with agency's policies and procedures.
Knowledge of basic ethics and privacy rights.
Ability to maintain professional boundaries with clients, the community, and other staff members.
Demonstrate judgment and discretion in dealing with confidential matters.
Commitment to Catholic Charities mission to work in partnership with vulnerable populations to achieve lasting solutions to poverty and injustice.
Proficiency in Microsoft Office programs (including Word, Excel, Outlook and PowerPoint).
Must be able to organize and prioritize work, be proactive, take initiative, follow through, and simultaneously manage multiple priorities to ensure goals are met in a timely manner. High attention to detail required.
Strong analytical and strategic problem solving skills.
Willingness to learn new skills and take on new responsibilities.
Ability to work well in team setting, as well as independently; be flexible and adapt well to different dynamics.
Satisfactory results from civil, criminal, and motor vehicle background check required
