Senior Services Case Manager

Community Strengthening Program

Full-time Position (40 Hours a Week) Non-exempt

Reports to: Community Services Program Manager

POSITION SUMMARY:

The Senior Services Case Manager provides culturally specific services to older adults and their family members living in Multnomah County. The case manager is responsible for providing case management, options counseling, information and referral, and transportation services practicing from a person-centered perspective and the assertive engagement model. Assertive Engagement (AE) is a social service approach that honors people as the experts in their own lives and draws from Strength Based Practice, Assertive Community Treatment, and Motivational Interviewing.



PRINCIPLE DUTIES AND RESPONSIBILITIES:



Case Management/Options Counseling

Conduct an assessment to identify needs, and in partnership with the participant develop service plan to address those needs.
Identify the appropriate referrals and services that meet participant's goals.

Work with participants in advocating for their needs (e.g. with health care providers, family members, medical settings, other organizations, etc.).
Meet stated monthly goals of case management and options counseling services
Conduct home visits as needed.
Enter data in a timely manner into Oregon Access (State of Oregon database), the Aging and Disability Resource Connection (ADRC), Caretool (Multnomah county database), and Salesforce (EPHC database).
Maintain confidential participant files in compliance with EPHC and Multnomah County requirements.
Participate in monthly file reviews, e.g. with other service providers.

Actively participate in team projects including team, agency, program and partner meetings.
Perform other duties as assigned.





Information & Referral and Transportation Coordination

Provide culturally specific referrals to seniors in person and over the phone.
Respond to callers and walk-ins following appropriate standards.
Enter information into the appropriate database while being on the phone.
Screen and coordinate transportation services for seniors living in Multnomah County.
Provide in-person and over-the-phone services and perform computer work throughout the day.

Adhere to EPHC policies and procedures.



QUALIFICATIONS:

Must be bilingual and bi-culturally competent (Spanish/English).
Excellent communication skills, both verbally and in writing in Spanish and English.
Knowledgeable about resources and services available for older adults.
Must have experience providing case management services.
Bachelor's degree in social work/human services preferred; and/or equivalent work experience.

Intermediate proficiency in programs of Microsoft Office; experience with data entry preferred.

Must successfully complete AIRS (Alliance of Information and Referral Systems) certification process within one year of being hired.
Demonstrate judgment and discretion in dealing with confidential matters.
Must be able to organize and prioritize work, be proactive, take initiative, follow through, and simultaneously manage multiple priorities to ensure goals are met in a timely manner. High attention to detail required.
Willingness to learn new skills and take on new responsibilities.
Strong analytical and strategic problem-solving skills.
Ability to work well in team setting, as well as independently; be flexible and adapt well to different dynamics in a fast paced work environment.

Ability to lift 40 pounds.

Must have driver's license, access to vehicle and ability to obtain automobile insurance at levels required by agency (100/300/100).
Successful completion of civil, criminal and motor vehicle background checks.
Ability to support EPHC mission and goals.
