CCC’s Blackburn Center is seeking a Case Management Operations Supervisor, QMHA or QMHP
Schedule: Monday-Friday, 7:30am-4:00pm
Blackburn Center is a new six-floor building in East Portland, Oregon (clinic and housing) that will give vulnerable people access to integrated housing, employment and health services, and provide support they need to stabilize and rebuild their lives.
The Recuperative Care Program Case Management Operations Supervisor works as a member of and supervises case management staff of a multi-disciplinary team providing intensive case management and care coordination for medically vulnerable adults facing homelessness upon discharge from the hospital.

Duties will include:
Work as a member of the Blackburn Center multi-disciplinary RCP team to deliver comprehensive, client centered and trauma informed integrated services.
Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn and RCP Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
Supervise and provide coaching to assigned Recuperative Care case management staff.
Assist the Clinical Program Manager and Associate Director in maintaining quality assurance systems for all tasks associated with program operation. Provide feedback and coaching to team members supporting the quality of their work. Provide additional training to Recuperative Care staff as needed to maintain program standards.
Monitor attendance and time management of Recuperative Care case management staff. Mentor team members on effective time management.
Work under the medical direction Central City Concern Health Services (CCCHS) to coordinate acute and ongoing medical care.
Provide transportation for patients in CCC vehicle to and from program related appointments, interviews, and hospitals/clinics.

Requirements:
Master’s degree in health, business, or public administration, health sciences, public health, or related field and at least 2 years of progressively responsible experience in clinical leadership, health care or social services operations, or health care business management OR Bachelor’s degree in health, business, or public administration, health sciences, public health, or related field and at least 4 years of progressively responsible experience in clinical leadership, health care or social services operations, or health care business management.
Must meet state guidelines for QMHA and be registered with the appropriate board.
QMHP certification is preferred.
Minimum two years direct supervisory experience required.
Must meet CCC privileging requirements as required by FTCA.
Must have current Healthcare Provider CPR/First Aid certification.
Must possess a current driver’s license, pass a DMV background check and be designated an “acceptable” driver as outlined in Central City Concern’s Fleet Safety Policy. Must pass an initial drivers training and annual recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
Must be available to work weekends and/or holidays and on-call hours when necessary for the program.
Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
