CCC's Old Town Clinic is seeking a Health Assistant to join the team Monday-Friday 8:00am-5:00pm!

SUMMARY: The Health Assistant (HA) works as a member of a multi-disciplinary primary care team providing integrated primary care services to homeless and low-income patients seen at Old Town Clinic. The Care Team is responsible for proactively managing the healthcare needs of the patients on their assigned panel, including behavioral and medical health needs. The HA is responsible for providing support to the Care Team Managers (CTM) and providers on each primary care team.

Duties will include:
Receive, document and direct patient telephone calls in a complete and timely manner according to clinic protocols.
Prepare prescription refills under provider orders.
Electronically file and index clinical information into the patient’s chart in EMR.
Obtains, processes, and documents all necessary information and paperwork required to assist the Care Team Managers (CTM) and providers in the provision of health services to Old Town Clinic patients.

High school diploma or GED required. Prefer Bachelor’s Degree or higher in health or social service related field of study
At least one year experience working with homeless, low-income, or other vulnerable populations in a supportive role preferred.
At least one year experience working in a clinic setting preferred.
Has the competencies necessary to:
a. Communicate effectively orally and in writing;
b. Understand healthcare related terminology;
c. Provide outreach, engagement and supportive services.
