The Wellness Navigation Specialist at Self Enhancement, Inc. (SEI) is a position for the Community Works Project (CWP) located on-site at the Department of Human Services East County Family Service Center. CWP is a six-agency collaborative project providing job preparation and placement services to Temporary Assistance to Needy Families (TANF) participants who are in Job Opportunities Basic Skills (JOBS) program of which SEI is a part of. CWP serves two main Department of Human Services (DHS) locations and participants are referred from DHS Family Coaches. The Wellness Navigation Specialist is responsible for coordinating the care plans for CWP participants facing; medical, mental health, and/or drug and alcohol barriers. The Wellness Navigation Specialist acts as a resource and advocate to connect participants to health and wellness services in a culturally responsive manner. This position will receive training in Community Health Work (CHW).


•Coordinate individual care plans designed by Medical Case Consultant and/or Behavioral Health Consultant through case management and communication with all parties, This includes meeting participants at their home, in the community and/or accompanying them to a service provider.
•Provide culturally-specific assessment of strengths and needs of participants enrolled in activities in coordination with Medical Case Consultant and/or Behavioral Health Consultant
•Act as a resource for direct service providers when working to support families, and share information with them to assure that services being provided are appropriate and coordinated.
•Monitor and document ongoing progress toward these goals and objectives.
•Provide a strength-based approach in all family engagement using motivational interviewing when appropriate.
•Provide support to individuals and families to resolve health-related issues that are negatively impacting wellbeing.
•Support a successful relationship between families and direct service organizations.
•Obtain all required medical information from providers and enter all received documentation in TRACS and ETO database systems.
•Build connections with local primacy care clinics (e.g. Mid-County Health Clinic, Providence Gateway, Rosewood, and Kaiser Gateway) and other providers, as time allows
•Participate in case reviews with other team members and coordinate connections with non-medical service providers as necessary
•Teach culturally-specific wellness promotion classes and provide one-on-one wellness promotion counseling to participants, using resources provided by Medical Case Consultant and/or Behavioral Health Consultant.
•Communicate with families at least twice a month to determine any potential barriers to success.
•Continue to engage with families as long as there is a need.
•Accepts other appropriate duties/responsibilities as assigned by SEI management.


•Bachelor’s Degree or equivalent in Education, instruction, Social Services, or a related field
•Experience with care coordination, case management, and/or experience working with participants and service providers in order to achieve participant objectives.
•Listening and empathy skills (i.e. compassionate communication) and ability to assist with problem solving and resolving conflict.
•Experience and ability to work in a multi-cultural, multi-ethnic diverse environment
•Ability to function in a positive manner in a demanding work environment, and demonstrate a high degree of flexibility responding to priorities and schedules that change frequently
•Must have effective interpersonal skills with the ability to interact with people of varying social, economic and education levels.
•Must have excellent motivation and problem solving skills with the ability to successfully connect with others.
•Must be highly organized and self-motivated with effective time management skills.
•Must have excellent computer skills; experience using MS Office, familiarity with using social media platforms
•Ability to use basic office equipment, telephone, copy machine, fax machine, and computer
•Must have current and valid state driver’s license, in good standing with no insurability restrictions and must be insurable under SEI’s auto insurance policy.
