CCC’s Blackburn Center is seeking a Community Liaison for the Recuperative Care Program!
Blackburn Center is a new six-floor building in East Portland, Oregon (clinic and housing) that will give vulnerable people access to integrated housing, employment and health services, and provide support they need to stabilize and rebuild their lives.
The RCP Community Liaison provides integral and embedded support to the Recuperative Care Program by acting as a front desk greeter for clients and visitors, including monitoring the door and access to RCP, providing social contact for active RCP clients and being a positive presence in the RCP milieu, and assisting clients with accessing RCP resources. This position is a unique blend of interfacing with clients served by the Recuperative Care Program and overseeing the program’s critical authorizations management system.
Duties will include:
Greet RCP clients and visitors at the RCP Front Desk.
Provide social contact for active RCP clients and assist clients with accessing RCP resources.
Work as the primary facilitator of RCP authorization processes with community partners.
Manage authorizations for new referrals, drive processes for reauthorizations over the course of program participants’ enrollments.
Act as the primary point of contact with funding partners on their respective authorization systems.
Gather medical records from multiple sources and substantial partnership with RCP’s
interdisciplinary team to understand program participants’ need for services.
Facilitate RCP authorization processes in collaboration with funding partners, RCP team, and billing department.
One year experience in a medical and/or social services setting preferred.
Has the competencies necessary to: Communicate effectively orally and in writing, understand medical related terminology, perform confidently with computer software
Must have an understanding of the causes of homelessness, the medical and
psychosocial sequelae of homelessness, and the systems and processes used to help clients secure and maintain stable housing.
Must have current Healthcare Provider CPR/First Aid certification or be able to obtain in 60 days.
Must have or quickly develop a working knowledge of medical terminology.
Must be able to be trained in the use of CCCHS Database programs CCCER and Centricity.
Must be able to occasionally lift items of approximately 10 pounds, stoop, kneel, and climb stairs on a regular basis.
Must have a high tolerance for ambiguity and change.
Must have non-violent de-escalation training (or complete within 60 days of hire.)
Must have excellent reading and comprehension skills.
Must pass a pre-employment drug screen, TB test, and background check for work at Central City Concern and to obtain ID-Badge within first 30 days of employment.
Must have non-violent de-escalation training, or complete within 90 days of hire.
Must possess a current driver’s license, pass a DMV background check and be designated an “acceptable” driver as outlined in Central City Concern’s Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
Must be available to work weekends and/or holidays and on-call hours when necessary for the program.
Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit and the ability obtain ID-Badge at Partner
