Under Oregon statute, after July 1, 1996, an offer of employment for this position is contingent upon the results of criminal offender information record check, and possibly fingerprinting, conducted on applicants for employment in early childhood services agencies. In support of the Agency’s mission, this position is responsible for promoting stability and self-reliance for low-income families and individuals by providing home visiting and/or in-office comprehensive case management services to clients. Duties include comprehensive assessment, goal setting, linkage to services, monitoring, education, advocacy, and evaluation. Duties also include establishing collaborative relationships with service providers and clients while achieving positive results with culturally diverse populations. For specific functions and responsibilities for each Family Advocate specialization, please see Essential Functions and Responsibilities by Specialization section below.
Essential Functions and Responsibilities of all Family Advocates:
• Provide case management to families in a manner that promotes self reliance and stability including: comprehensive assessment; goal planning; linkage to services and resources; monitoring service delivery; advocacy for and with clients; evaluation; and effective disengagement.
• Work in collaboration with Community Action staff and other agencies and partners to coordinate services and resources.
• Ensure that client records are maintained in a manner that guarantees confidentiality and accurately reflects assessment, services and outcomes.
• Enter data into the Homeless Management Information System (HMIS), Service Point, Family Manager, and/or other database(s) as necessary.
• Compile statistical information as needed for program reporting and evaluation.
• Measure, track and report outcomes related to program goals, and overall family self-sufficiency.
• Maintain current information on community resources and applicable laws and provide assistance to families and individuals in accessing community resources.
• Attend staff meetings and trainings as required.
• Strictly adhere to all safety policies and procedures at all times and respond/correct any at-risk safety issues immediately.
• Strictly adhere to agency code of ethics and standards of workplace behavior.
• Perform other duties as assigned to meet business needs including regular and reliable attendance and adherence to all company workplace behavior standards.

Additional Essential Functions and Responsibilities for SSVF Specialization:
FAMILY ADVOCATE, SUPPORTIVE SERVICES FOR VETERAN FAMILIES (SSVF) – reports to Family Advocate Supervisor
• Provide home visiting case management services to Veteran families and individuals in a manner that promotes self-reliance and stability, including but not limited to: comprehensive assessment, goal planning, linkage to services and resources, monitoring service delivery, advocacy for and with clients, evaluation, and effective disengagement.
• Maintain regular contact with Veteran families and individuals to create goal plans and assist them in accessing necessary community resources.
• Minimum of a High School Diploma or equivalent with a minimum of one year of case management experience, including maintaining accurate and complete case files and coordination of services with other agencies. An equivalent combination of education and experience may be considered.
• Ability to work independently and produce quality work under tight timelines.
• Excellent written and verbal communication skills.
• Ability to set and maintain positive and professional boundaries with clients.
• Ability to establish trusting relationships with clients and staff, demonstrating acceptance of individual differences.
• Willingness to participate in reflective practice (demonstrates a capacity for introspection and an awareness of self in relation to others, recognizes the value of feedback and supervision, etc.).
• Ability to work effectively and collaboratively with a diverse client and peer population, including experience working with vulnerable and/or low income populations.
• Intermediate computer literacy and familiarity with applications such as databases and spreadsheets, word processing, email and internet.
• Available transportation and proof of auto insurance.
• Bachelor’s Degree in Social Services or a directly related field.
• Bilingual in English/Spanish; intermediate written and verbal competency.
• Knowledge of Washington County social services and resources including services specific to the target population being served (such as Veterans Administration services, employment services, 0-3 services, etc.).
• Prior Community Action program experience.
• Current or former experience as a veteran or as a direct service provider to veterans.
Required Licensing or Other Special Certifications:
• Must have a valid driver’s license and maintain a good driving record.
Required Physical Demands of Essential Functions and Responsibilities:
• Work is performed in normal office environment and/or client’s homes.
• Moderate computer and phone use.
• Occasional early morning, evening or weekend work as needed.
• Local driving in various traffic and weather conditions to provider’s facilities or homes using personal vehicle.
• In normal work day, may stand/walk __2.5__ hrs/day; may sit _3.5_ hrs/day; may drive _2.5_ hrs/day; may use hands for repetitive grasping, pushing and pulling, typing and fine manipulation _1/2_ hrs/day; may use hands at or above shoulder level for _1_ hrs/day.
• In normal work day, may bend, squat, and/or climb frequently.
• Work is light in nature, lifting maximum of 20 lbs with frequent lifting and/or carrying of objects weighing up to 10 lbs.
*Note on Hiring Range:
For applicants with extensive qualifications and/or years of experience, the new hire maximum rate may be exceeded with the approval of the Director of Human Resources and the Executive Director. If approval is granted, it will be based upon careful review of the applicant’s qualifications, experience, internal equity and budgetary considerations. The final rate approved will typically never be at a rate equal to or above the established market rate for the position.
